Sports Medicine Clinic

Evanston Township High School Athletic Training is proud to partner with Endeavor Health to offer a weekly, in-house Sports Medicine Clinic at the School-Based Health Center—completely free of charge to student-athletes.

About the Clinic

Every Monday afternoon, our team physicians from Endeavor Health are on campus to evaluate in-season student-athletes who need further medical assessment for an injury. The clinic is held in the ETHS School-Based Health Center (H101).

Scheduling

Appointments at the Sports Medicine Clinic are prioritized based on the severity of injury and type of visit. All appointments must be scheduled through the ETHS Athletic Training staff.

  • Clinic evaluations and in-clinic follow-ups are free of charge.

  • If the student has seen an outside provider, please share any medical notes or imaging reports with the athletic trainers in advance. These can be copied and given to our team physicians during the visit.

  • Please note that our in-house clinic does not offer imaging services such as X-rays or MRIs. If imaging is needed, the ETHS Athletic Trainers can assist in scheduling a follow-up appointment with our team physicians at their Endeavor Health offices. Note: Medical appointments outside the clinic will require medical insurance.

Parent/Guardian Consent for Minors (Under 18)

A parent or legal guardian must sign a consent form for students under 18 years old to receive services at the ETHS School-Based Health Center.

Student 18 and Older

Students who are 18 years or older can provide consent for all medical services at the ETHS Health Center on their own. Parent/guardian consent is not required.
