Student-Athlete Forms
All ETHS student-athletes are required to have certain forms completed and submitted before participating in practices, camps, tryouts, or games. Please review the list below and ensure your documents are current and on file.
All student-athletes are required to have a current physical exam on file to participate in athletics at ETHS. This exam must be documented using the State of Illinois Child Health Certificate form, which is the standardized form accepted by schools across Illinois. The physical must be completed by a licensed healthcare provider.
The following procedures support the Student-Athlete Concussions and Head Injuries policy (Board Policy 7:305) and are designed to ensure that concussed athletes are properly identified, treated, and referred according to current evidence-based standards of care. This protocol also helps ensure that students receive appropriate support during the school day—including academic accommodations when needed—and are fully recovered before returning to physical activity.
Student-athletes are expected to travel with their team to and from all away contests. A parent/guardian may transport their child from an away contest only if a completed and signed Transportation Release Form is submitted directly to the coach before departure.
If you are a transfer student and plan to participate in athletics at ETHS, you must complete the IHSA Principals’ Concurrence Regarding Transfer form. This form is required by the Illinois High School Association (IHSA) to verify that your transfer is in compliance with IHSA eligibility rules. It must be completed by both the sending and receiving schools and approved by the principals. Student-athletes may not participate in practices or competitions until this form is submitted and approved.
