Register for Sports

The Baseline ImPACT Test (Immediate Post-Concussion Assessment and Cognitive Testing) is a computerized neurocognitive exam used to help evaluate and manage concussions in student-athletes. ETHS requires all incoming freshmen and new student-athletes to take a baseline version of this test before participating in any sport. Taking this test is one of the many ways ETHS ensures the safety and long-term health of its athletes.

The baseline test does not involve physical activity—it’s completed online in a quiet environment using a computer. The results establish a personal baseline for each student’s normal brain function when they’re healthy. If the student ever sustains a head injury during the season, this baseline can be compared to a post-injury test to help determine the severity of the concussion and guide the return-to-play process.

Students must complete the ImPACT test at least 30 days before the start of their first sports season at ETHS.

*This test is required only once for all student-athletes. Returning athletes who have already completed the ImPACT test do not need to retake it.

How to Take the ImPACT Baseline Test

Before You Begin

• Use a laptop or desktop computer (phones and tablets are not supported).
• Choose a quiet, distraction-free space with a stable internet connection.
• Set aside 30-45 minutes to complete the test in one sitting.

How to Take the Test (Students)

1. In your web browser, go to www.impacttestonline.com/schools
2. When prompted, enter the activation code: 49fe191b59
3. Click the button reading "Launch Baseline Test."
4. Enter your full name and follow the on-screen instructions to start the exam.
5. After completing the exam, save or print the confirmation page for your records.

What Happens Next

• Please allow 2–3 business days for our Sports Medicine staff to manually upload your results into the ETHS system.

• If your confirmation page lists the Test Type as “Baseline +” or “Baseline ++”, please contact our Athletic Trainer Khaliah Elliston at ellistonk@eths202.org for next steps.

A parent/guardian should log into myETHS using their myETHS credentials (same as HAC login information). For steps on how to login to myETHS, please visit eths202.org/families/parent-tools.

Note: Incoming freshmen families will receive their login credentials in late May.

Once logged into myETHS, go to the "Records" section and click on "Athletic Registration" to provide consent for the following:

Athletic Registration --> Select YES
Parental consent is required for students to participate in any ETHS sports program. Consent confirms that the parent/guardian agrees to the student's participation and acknowledges district policies. Consent can only be provided through a parent/guardian myETHS account; students cannot provide this consent themselves.

Media --> Select YES (if opting-in)
Under FERPA (Family Educational Rights and Privacy Act), ETHS cannot include a student's name or photo in team photos, rosters, videos, and promotional materials if the student is on the district's Media Opt-Out list. To provide media consent, a parent/guardian must select "YES" in the Media category during online enrollment or when updating registration information. Parents/guardians can view or change their student’s media consent status in myETHS at any time.

A sports physical is a health exam performed by a licensed healthcare provider to make sure a student is healthy and medically cleared to participate in athletic activities. A current and valid sports physical is required for participation in any sport. Students cannot practice or compete without a physical exam on file.

The physical must be completed on the State of Illinois Child Health Certificate form by a licensed medical provider and submitted to the Athletic Office before participation. This form can be used for school enrollment and sport registration.

The IHSA Sport Physical form can ONLY be used for sport registration and CANNOT be used for school enrollment. 

Check Your Current Physical

A student's current physical exam date can be viewed in myETHS under Records > Athletic Registration. A physical is valid for 365 days (1 year plus 30 days). If the physical on file is older than this, you will need to submit an updated exam before the student can participate.

How to Submit Your Physical

Submit physicals at least two weeks before your sport's start date to allow 5 business days for processing:

Email	Email a scanned copy (photos are not accepted) to: sportsphysicals@eths202.org.
Fax	Fax your physical to (847)492-5806.
Drop Box	Drop off a paper copy in the secure drop boxes located at Entrance 1, Entrance 2, or the Nurses’ Office (Room N121). Be sure to use the provided sports physical envelope at the drop box to ensure your form is processed promptly.

Physical Exams at ETHS

The ETHS School-Based Health Center (operated by Endeavor Health) offers sports physicals by appointment. To schedule a physical exam for your student, call 847-424-7265 or email healthcenter@eths202.org. For more information, including cost and insurance information, visit eths202.org/families/school-based-health-center.

Note: Students under 18 must have a signed Consent for Services form from a parent or guardian on file in order to receive a physical exam.

When logged into myETHS, go to Athletic Registration, select the season, and then choose your sport. Click the green button labeled “Register for Selected Sport.”

If your student plans to participate in multiple sports throughout the school year, you must register for each sport separately, selecting one season and sport at a time.

Note: If the button appears red instead of green, it means your student's sports physical is expired or Parent/Guardian Consent has not been submitted. Please update those items before proceeding (see steps above).
